This Saturday, April 27th, is National Prescription Drug Take-Back Day.  

Several local Police Departments and law enforcement agencies will be participating in the U.S. Drug Enforcement Administration’s (DEA) initiative.  This nationwide drug take-back day brings attention to the issue of pharmaceutical controlled substance abuse.

The DEA website states “The National Prescription Drug Take-Back Day aims to provide a safe, convenient, and responsible means of disposal, while also educating the general public about the potential for abuse of these medications.”

From 10am to 2pm, our local heroes will be operating collections sites for safe disposal.

While one knee is kneeling on a towel or soft cushion, the other leg is bent forward in front of you. Keep your abdominal muscles in a contracted position while you slowly move forward and stretching the hip. Hold the stretch for a count of 3-5 seconds before returning to starting position. Doctor Becomes Patient: Back to the Bike

In previous posts, I had discussed an ACL rupture that occurred to my right knee in 2011.  Although I have recovered to near full strength and have been able to return to running, basketball and other active sports, I also understand that based on clinical studies, the risk of osteoarthritis is high in individuals who have had ACL rupture, regardless of whether surgery is performed or not. 

What is Osteoarthritis?

In light of this evidence, I have ventured into the world of road biking.  Generally speaking, cycling causes less impact on the joints than running does.  Initially, I contemplated getting a true road bicycle, which allows the rider to adopt a more streamlined approach.   However, in the end, I purchased a hybrid bicycle.  This type of bike is best ridden on the roads, has the thinner “road tires” also known as 700mm diameter, and has flat handlebars.  The geometry of the frame combined with the flat handlebars allows the rider to sit in a more upright position.  I decided to go with this hybrid type of bicycle for three main reasons:  (1) lower cost (2) no compromise in terms of fitness potential (3) more upright position.  

Most people would agree that exercise has benefits.   In general terms we have the understanding that exercise is “good for the heart” and helps people “lose weight.”   However, when it comes to our own personal health and ailments, we sometimes have doubts as to whether exercise is truly feasible.   Moreover, it can be daunting to start a new exercise program without the knowledge of what is a realistic goal. 

In a nutshell, people are classified as meeting exercise recommendations if they report engaging in moderate-intensity activity at least 30 minutes per day, 5 or more days per week, or vigorous-intensity activity at least 20 minutes per day, 3 or more days per week.  The American Heart Association describes moderate-intensity activity as raising the heart rate noticeably such as a brisk walk.  Vigorous-intensity activity for most people would be the equivalent of jogging, which raises heart rate substantially and causes rapid breathing. 

In the next few posts, I would like to provide you with basic tools to create an exercise program.  Setting goals, which are specific, clear, and moderately difficult to achieve, are most likely to motivate.  Although this blog is meant to inform, it should not be taken as a personalized exercise prescription plan.  If you are not already in the habit of exercising, please speak with your doctor and health care professional prior to embarking on a new program.

This exercise works the abdominal, oblique internal/external, iliopsoa muscles.

The purpose is to increase abdominal strength and muscular endurance.

Begin lying on floor.

Lift knees so that a 90º position is attained at hip and knees. Place hands beside ears. Activate core.

Lift shoulder off floor approximately 6 inches attempting to bring elbow toward opposite opposite knee so that both simultaneously meet.

Return to start position and repeat on opposite side. Continue alternating sides until recommended repetitions are complete.

Sciatica Treatment with Lysis of Adhesions

Performance Spine & Sports Medicine offers an innovative treatment for sciatica called “lysis of epidural adhesions”. The name implies that there is scar tissue in the epidural space and it is approved for the treatment of scar tissue. However, it can also be used in patients that have not had back surgery. Indications include failed back surgery syndrome, sciatica, low back pain, pinched nerve, herniated disc.

Lysis of Epidural Adhesions involves placement of a specialized catheter into the epidural space. We then inject contrast dye to visualize the defect that is causing the sciatica or low back pain. Often this is due to scar tissue or a herniated disc. Medications are then injected to soften these structures which allows specialized exercises to be performed in order to free the nerve root. This often gives tremendous relief of the leg pain and normalized function.

See the images below. The first image shows contrast dye throughout the spine but a lack of flow on the left side. This patient had sciatica involving the left leg. Note that the second image shows an opening of this space with flow down the nerve root. This patient had significant relief of left leg pain and restoration of function.
